Age 48, Loving Wife and Mother

Lost her year long battle with lung cancer on October 1st, 2011. Preceded in death by her father,  Robert Horak; mother, Elaine Schneider; infant son,Paul Formanek; and father-in-law; John Formanek. Survived by husband of 27 years; Jack Formanek; daughters, Danielle (John) and Randi Formanek; siblings, Sue, Barb, Pam, Terri, and Rob; a large extended family of cousins, nieces, nephews, in-laws, and a world of close friends.  She loved and was loved by many who will miss her greatly. Memorial Service Saturday, 10:00 A.M. at EAGLE BROOK CHURCH, 2401 East Buffalo St. & HWY 61, White Bear Lake. Visitation  Friday, 4-7 P.M. at BRADSHAW, 4600 Greenhaven Drive at Hwy 96 (one mile west of 35E), White Bear, as well as one hour prior to the service at the church. Memorials preferred to the family (will be donated to Eagle Brook Church and the University of Minnesota Medical Center).
